Note: In many states, you must contact the operator first and try to resolve the complaint with them before filing a dispute with the governing body. The below contacts and complaint forms are to be used after that path has been pursued.

It is important to maintain accurate records of all actions and conversations, including date and time, nature of the complaint, participants in the conversation, next steps, and the like.
